Tuition & Fees-Related Forms

  • FAFSA—To receive financial aid, work study and/or scholarships, you must complete this application. For Spring, applicants should have already completed this form. If you have not done so, please apply immediately.  Also, the priority deadline for the 2019-2020 application is January 31st.   The school recommends that all students complete the FAFSA. Our school code is 002104.

  • Tuition Classification Form (In-State Residency)—All admitted students are classified as “out-of- state” for tuition and fees.  To qualify for “in-state” status students must submit the In-State Classification form. This form should be submitted no later than December 15th. Submit Forms to registrar@umaryland.edu, Fax—410-706-4053, or mail them to: Office of the Registrar, 601 W. Lombard Street, Ste. 240, Baltimore, MD 21201.
  • Health Insurance Waiver—Students who do not wish to retain the Student Health Plan must file a waiver each academic year to have the charge removed from their tuition bill. This form verifies an alternate insurance policy (and waives automatic enrollment in the University-sponsored program). If the form is not received by February 15th, students will be enrolled in the University plan and cannot receive a refund.
